Use this form to certify that you are eligible for an increased annual contribution limit as a result of your earned income.

Initiate a rollover from another qualified ABLE plan to an existing NY ABLE plan account.


Payroll Direct Deposit Form

Use this form to:

  • Deduct money automatically from a paycheck and deposit it directly into the NY ABLE account (the employer must offer this benefit).
  • Change the existing deduction.
