Incoming Rollover Form
Initiate a rollover from another qualified ABLE plan to an existing NY ABLE plan account.
Payroll Direct Deposit Form
Use this form to:
- Deduct money automatically from a paycheck and deposit it directly into the NY ABLE account (the employer must offer this benefit).
- Change the existing deduction.
